Register

All women who receive cervical screening are eligible to register for the CSIS. Registration is free of charge.

Benefits of joining CSIS:

• Review submitted screening records on the Cervical Screening Information System (CSIS)
• Receive reminders for cervical re-screening by email, by SMS or by post
• Authorise selected service providers to review participant’s screening records for better and continuous care

Registration Methods:

1. Online at CSIS website (www.csis.gov.hk) or
2. By Post or
   • The completed Registration form and a copy of your HKID/ passport should be mailed to the Cervical Screening Programme Office:
   • Cervical Screening Programme Office, Department of Health
   • Room 10, 5/F, 134 Queen's Road West, Sai Ying Pun, Hong Kong
3. Through registered Service Providers
